Dr House på neurologen

Den här veckan har jag VFU på avdelning. 4 ynka dagar har vi där två dagar spenderas på strokavdelningen och de andra två på den "vanliga" neurologiavdelningen. Anledningen till att jag sätter citationstecken över vanliga är att patienterna som ligger på Sahlgrenska ganska sällan har de vanliga sjukdomarna eftersom dessa oftast kan skötas på respektive hemortssjukhus. Den ena sidan av avdelningen beskrevs av vår avdelningsamanuens som det närmaste Dr House man kan komma där många av patienterna har väldigt atypiska sjukdomsvarianter eller symtom där t.o.m. överläkarna rynkar pannan och patienterna får utredas med många utredningar och prover man själv aldrig hört talas om i jakt på en diagnos.

 

Jag började i alla fall min vecka på stroken och där blev vi försedda med en strokesökare. Vi hade verkligen tur (om man nu kan kalla det tur) att vi under våra 2 dagar där hann springa iväg på inte mindre än 5 strokelarm. Det var väldigt intressant att bevittna det akuta omhändertagandet och vi fick till och med följa med en patient upp till operation för en akut trombektomi där man går in i blodkärlen med en kateter i ljumsken, letar sig upp till hjärnans blodkärl och drar ut proppen manuellt. Eftersom operationen sker inuti kärlen kan operatören givetvis inte se hur hen skall styra katetern och därför sker hela operationen med röntgengenomlysning.

 

Här har ni yours truly iförd blydräkt och blyhaklapp (förlåt Birgitt för att jag lånade din, vi kandidater kan inte göra något rätt tydligen) som skydd för röntgenstrålningen. På den högra bilden står några av mina kurskamrater och tappert försöker förstå en patients perfusionsbilder (fråga inte, jag vet inte heller riktigt vad det är!).

 

 

Idag har jag istället varit på Dr House-sidan på neurologiavdelningen. Syftet med vår placering på avdelningarna är att vi skall träffa så mycket patienter som möjligt och helst av allt vill ju vi kandidater få se så många sjukdomar som möjligt för att få ett ansikte på allt vi läser i böckerna. Då är det minst sagt lite klurigt när mer än hälften av patienterna har oklara eller atypiska sjukdomar. Visst är det intressant att få se alla sällsynta sjukdomstillstånd men det är ju med största sannolikhet inte dem vi kommer stöta på i vår framtida karriär. Imorgon skulle vi förhoppningsvis få träffa en parkinssonpatient i alla fall. Skall bli spännande att se om symtombilden i boken stämmer överens med det vi får fram hos patienten!
